Social prescribing is when health professionals refer patients to non-clinical support services in the  community in order to improve their health and well-being. In the US, many healthcare providers have  begun to institute social prescribing in a myriad of ways and on countless technology platforms. They are  using an array of needs assessments (such as mental health, social determinants of health) to drive  community referrals for food, housing, transportation and more. 

Social determinants of health (SDOH) are conditions in the environments where people are  born, live, learn, work, play, worship and age that affect a wide range of health, functioning and  quality-of-life outcomes and risks.

Neighbors in Health is an existing program within Horizon Blue Cross Blue Shield of New Jersey that  employs community health workers to assess members’ social determinants of health and connect them  with local resources accordingly. By definition, Neighbors in Health is a social prescription model. Individuals who are Horizon Neighbors in Health members, aged 18+, who live in or can easily access Newark and/or Essex County are eligible for the ArtsRx program. 

As an ArtsRx participant, you receive free access to arts and cultural activities throughout Newark. ArtsRx members will never pay for an ArtsRx activity. All costs are covered by NJPAC Arts & Well-Being, thanks to generous support from RWJBarnabas Health System, the Horizon Foundation of New Jersey, Women@NJPAC, and other founding supporters. And you never have to go alone, you can bring up to three friends, family members or caregivers. We will allow children to come as companions only if the art activity session is age appropriate.
